Please arrive 15-20 minutes early for your appointment, this will give you an opportunity to browse our selection of frames at our on-site optical gallery.
If you need to cancel or reschedule your appointment, please give us at least 24 hour notice.
Insurance & Payments
Co-payments/Deductibles/Outstanding Balances are expected at the time services are rendered.
Payments for materials (eyeglasses and contacts) are expected to be paid in full at the initial placement of the order.
Additional Information
Please bring a current copy of your insurance card and a up-to-date list of any medications you may be taking.
